Office Policies for Ashley Burke, LPC

• Session fee: $150 for a 50 minute session

• Phone contact between sessions with clients or other professionals:

  • Any time spent in excess of 10 minutes will be billed a pro-rated amount based on the hourly rate

  • Please note that this contact is not eligible for reimbursement from your insurance company

• I do not bill directly to your insurance company, but do provide an invoice to you with all necessary information so you can file for an out-of-network reimbursement. Please check with your insurance policy for your out-of-network benefits.

Cancellation of Appointments

In order to best serve all of my clients and to be able to accommodate those on a waiting list, clients must give at least 24 hours notice when it is necessary to cancel an appointment. Cancelling with less than 24 hours notice or not showing up to an appointment will result in a charge of the hourly rate for the missed appointment.
